Erin Denholm is the President and CEO of Trinity Home Health Services in Livonia, Michigan. Supporting  home based programs in 21 states, Ms. Denholm is leading one of the top 10 largest home care organizations in the country. Previously Denholm served for 20 years in Centura Health most recently as SVP of Clinical Transformation for Centura Health. A Robert Wood Johnson Nurse Executive Fellow, she also served as one of 72 CMS Innovation Center Advisory Council members. She is a distinguished leader in healthcare, noted for her visionary and passionate commitment to transcend healthcare beyond hospital walls, innovate new models of care that improve quality while decreasing costs, increase healthcare literacy among consumers and, in particular, empower consumers with the resources to direct their care. She pioneered the use of telehealth technology in reducing emergency room visits and adverse outcomes among heart failure patients in Colorado, and led the successful legislative effort to make Colorado the first state in the country to provide Medicaid funding for telehealth services.
 
Ms. Denholm has won numerous accolades for her work. Under her auspices, Centura Health at Home had been recognized as a world-class workplace by the Gallup organization. She is a recipient of The Denver Business Journal Champion in Healthcare award and her collaborative spirit inspired a joint venture with The Children's Hospital to fund and operate the first palliative care program in Colorado designed exclusively for children and their families which has received national acclaim. She is an international speaker and consultant on telehealth nursing and post -acute care leadership. Most recently Ms. Denholm co-authored a chapter in Nursing Policy and Politics, which won the 2015 ANA book of the year. Ms. Denholm holds an MSN from Regis University and a BSN from The College of Mt. St. Joseph. She currently serves on the boards of the Colorado Center for Nursing Excellence. She Historically she has been on the Home Care Association of Colorado Board of Directors, The Colorado Hospice and Palliative Care Organization and has served on the Board of Directors for the Rocky Mountain Children's Choir.
